Clinical profile

Intravenous NAD⁺ is a popular "cellular recovery" procedure in wellness clinics. Oral NAD⁺ precursors (NR, NMN) reliably raise NAD⁺ levels and are well tolerated, but for intravenous NAD⁺ for anti-ageing or wellness there are no clinical outcome studies; 2026 reviews call the evidence preliminary.

Every statement links to a publication in the list below. This is a snapshot of published research, not a final assessment of the method: international practice and integrated protocols often run ahead of official publications. Draft based on published research; requires review by a physician. Revision: 2026-10-17.

Mechanism of action

  • NAD⁺ levels decline with age, which is the rationale for repletion strategies. [1]
  • During intravenous infusion (3 µmol/min), NAD⁺ is cleared from plasma rapidly and completely in the first 2 hours; metabolites point to enzymatic breakdown. [4]
  • The response to NAD⁺ is non-linear: it depends on dose, age, metabolism and route of administration, with feedback and saturation. [3]

Where studies show an effect

  • Heart failure in ischaemic cardiomyopathy: in an RCT (180 patients), 7 days of IV NAD⁺ added to standard therapy improved cardiac function. [5]
  • Oral precursors raise NAD⁺ and are safe; individual RCTs showed increased muscle insulin sensitivity in women with prediabetes and aerobic endurance in runners. [6, 8, 9]

Open questions and study limitations

  • No clinical studies of intravenous or intramuscular NAD⁺ for anti-ageing and wellness have been published yet. [1, 2]
  • In humans, the effects of precursors on functional, metabolic and vascular measures are inconsistent and often absent. [1]
  • The optimal dose, duration of effect and long-term safety of intravenous NAD⁺ have not been established. [2]

Safety

  • Oral NR and NMN were well tolerated in RCTs; NMN up to 900 mg a day. [6, 7]

Protocol in studies

  • In the heart failure RCT: 10 mg NAD⁺ IV daily for 7 days. [5]
  • Pilot pharmacokinetic study: a 6-hour infusion at 3 µmol/min. [4]
Contraindications. Contraindications are determined by a doctor; there are no clinical study data on contraindications for intravenous NAD⁺.

Evidence base

For intravenous NAD⁺ for wellness purposes there are no clinical outcome studies; there is one RCT in heart failure and RCTs of oral precursors with mixed results.

Publications were found in PubMed; titles and journals are given verbatim, conclusions are a brief summary of the abstracts under the conditions of the specific studies. The level shows the volume of published data, not the value of the method: A — Plenty of data: consistent systematic reviews and meta-analyses of RCTs; B — Some data: reviews or several RCTs, results are heterogeneous; C — Little data: single RCTs or controlled studies; D — Scarce data: mainly reviews, observational and experimental studies. The passport is updated as new work is published and with regard to international practice.
